Provides financial assistance to dialysis and transplant patients who are unable to pay for treatment-related needs. This includes:

Also provides information on kidney disease, treatments, and how to manage the disease in daily life.
Eligibility
Open to low-income dialysis or kidney transplant patients with referral from a medical social worker or physician.
