Eligibility
Individuals and providers who have questions, need help or need to report a problem about Medi-Cal.
Application Process
Call, must have the Medi-Cal Member's beneficiary ID number or the last four digits of the Member's Social Security Number AND the Member's date of birth (two-digit month and four-digit year ex: 12/1961)
