Description
Provides vouchers for income eligible applicants to purchase prescription eyeglasses.
Eligibility
Open to individuals ages 18-55
Applicants must have an income at or below 250% of the Federal Poverty Level
Application Process
Complete application online, If the application is approved, a unique e-voucher number is emailed with a link to agency's eyeglass ordering site and instructions on how to order eyeglasses
Documents
Proof of income
Applicants must have an eyeglasses prescription dated within the last 24 months, including their pupillary distance
Fees
$10 application fee
