Program: MULTI PURPOSE SENIOR SERVICE PROGRAM (MSSP)

Agency: VENTURA COUNTY AREA AGENCY ON AGING (VCAAA)

Description

Offers case management to seniors who want to remain at home instead of living in a nursing home. A care manager makes monthly calls or visits to clients. They advocate and coordinate the following services:

Grab bars
Respite care
Rides to appointments
Emergency response support services
Other independent living related services

Eligibility

Must:
-Live in Ventura County
-Be 65 years of age or older
-Meet the current US Federal Poverty Level Guidelines
-Have a possible need for nursing home care and be on Medi-Cal with no share-of-cost (SOC)
