Program: SENIOR NUTRITION PROGRAM - HOME DELIVERED MEALS

Description
Offers home-delivered meals to seniors 60 years of age and older. Meals are hot and include bread, salad, fruit, vegetables, and yogurts. Meals are delivered weekly or bi-weekly depending on how much space the client has to store the food. Staff also assess additional needs clients may have and provide referrals to nutrition-related supportive services.
Eligibility
Must be 60 years of age or older, or have a disability.
Application Process
Call to schedule a home visit. Applicant must complete an assessment form to determine eligibility. Waiting list may vary according to participating sites.
