Description
Enhances the quality of life for terminally ill patients and their families. Services include in home hospice care. Provides a team approach that ensures consistency and continuity in the quality of care. Each member of the team contributes their expertise to meet the patient's needs. They develop a unique plan of care. This is a Medi-Care certified hospice program.
