Provides a consultation between the family member/caretake and an Ombudsman who will introduce you to the world of long term care. Provides information on:
- Skilled nursing facilities
- Residential Care Facilities for the Elderly - large Assisted Living and small Board and Care homes
- Adult Day Health Care Facilities
Eligibility
Family member, friend or caregiver of individuals who is facing chronic illness, disability, or death.
