Descriptions
Offers a preventive program that delivers health assessments and services to low-income children. Health assessments are provided by enrolled private physicians and community clinics. Provides the following during assessment:
- Immunizations
- Physicals
- Dental exam
- Vision exam
- Hearing exam
- Lead/anemia/TB exams
Provides care coordination to assist families with:
- Medical appointment scheduling
- Medical transportation
- Access to diagnostic and treatment services
Eligibility
Low-income children and youth.
Application Process
