Description
Provides hospice support services to individuals experiencing short or long term disabilities including:
- Minimal assistance with burial expenses
- Provides a medical van that takes patients to doctor visits, and medical facilities
- Assessment of additional needs
- Caregiver training and classes

Individuals experiencing short or long term disabilities
Application Process
Call to make an appointment and reserve your medical equipment and incontinence supplies. Medical equipment is to be picked up and dropped off in the back of the facility. All equipment must be sanitized prior to drop off.
