Descriptions
Helps low-income elderly and people with disabilities stay in their homes by helping them pay for support services. Services include:
- Light domestic help
- Non-medical personal care
- Assistance to and from medical appointments
Eligibility
Must be:
-SSI/SSP or Medi-Cal eligible
-Blind, disabled or 65 years of age or older living in a home, apartment or residence of choice (not including a hospital, nursing home, assisted living or licensed care facility) and unable to live safely at home without care
Application Process
Individuals who already have Medi-cal, can call or visit to submit an application or fax it. Once application is received, a social worker will go to home to conduct an assessment of your mental and physical abilities and limitations to determine whether applicant qualifies.
