Program: SJRMC - DIABETES EMPOWERMENT AND EDUCATION CLASS

Description
Offers six-month clinic designed to help each client control their diabetes. Provides the following screenings for free:
- HbA1C
- Height
- Weight
- Lipid panel
- Blood pressure
- Blood glucose
- Body mass index
Also offers nutrition classes to participants. They can meet with a public health nurse to make plans towards their desired health goals.
Eligibility
Must be 18 years of age or older with pre-diabetes, and type 1 and type 2 diabetes
