Description
Provides support services to caregivers of older adults. Services include:
- Individualized consultations
- Respite care
- Home modification grants
- Community training and education
- Assessment of current caregiving challenges
- Review of medical, legal, physical, social and financial concerns
Also offers support group that provides a place for caregivers to discuss and resolve problems and determine needs.
Eligibility
Must be a caregiver of an older adult
