Description
Provides meals designed for specific medical needs and nutrition counseling for Gold Coast Medi-Cal members who have recently been hospitalized or have chronic conditions that could benefit from medically supportive food.
This is a CalAIM Community Support service under the CalAIM program.
Menu selections:
Four-week menu cycle
Customizable meals
Refrigerator/Freezer Requirement:
Refrigeration to store up to six meals at once
Medical Meal Options:
Diabetes
Kidney disease
Heart health
Cancer
Celiac/gluten-free
Reduced sodium
Soft/texture-modified
Vegetarian/Vegan
Weight loss
Weight gain
Delivery:
Provider staff delivers three times per week (typically four to six meals per delivery)
Additional information:
Local Ventura County medically tailored meal provider.
Provides meals for members 1 year of age and older.
CalAIM Community Support Service - Medically Supported Food
Eligibility
-Open to Gold Coast MediCal members who had a hospital stay for diabetes or Congestive Heart Failure-related reasons within the past 30 days.
-Must have service authorization from Gold Coast Health Plan Enhanced Care Management program
Application Process
Medi-cal beneficiaries can reach out through Gold Coast or directly to the provider to self-refer. A PCP or case manager can also refer a member.
