Provides meals designed for specific medical needs and nutrition counseling for Gold Coast Medi-Cal members who have recently been hospitalized or have chronic conditions that could benefit from medically supportive food.

This is a CalAIM Community Support service under the CalAIM program.

Menu selections:
Total of 120+ options
Customizable meals

Refrigerator/Freezer Requirement:
Refrigeration to store up to six meals at once

Medical Meal Options:
DASH/Heart healthy
Heart failure
Heart healthy diabetic
Heart friendly renal
Heart friendly renal diabetic
Gluten-free
Pescatarian/Vegetarian
Texture modication: Puree/Chopped

Delivery:
Provider delivers 14 meals per week covering two meals per day

Additional information:
Local Ventura County medically tailored meal provider.
Provides meals for members 1 year of age and older.

CalAIM Community Support Service - Medically Supported Food

Eligibility

Must reside in Ventura County
Must have active Medi-Cal insurance through Gold Coast Health Plan
Must not be receiving meals from other providers
Must have chronic or serious health conditions that are nutrition sensitive such as but not limited to high blood pressure, high cholesterol, congenital heart disease, obesity BMI is >30, diabetes, chronic kidney disease, fatty liver disease, cirrhosis of the liver, liver disease, cancer, strokes, malnutrition, also mental health like PTSD, bipolar, schizophrenia, etc.
