Description
Provides meals designed for specific medical needs and nutrition counseling for Gold Coast Medi-Cal members who have recently been hospitalized or have chronic conditions that could benefit from medically supportive food.
This is a CalAIM Community Support service under the CalAIM program.
Menu selections:
160 core options (700 total choices)
Customizable meals
Refrigerator/Freezer Requirement:
Refrigeration to store up to six freshly made meals at once
Medical Meal Options:
Heart friendly
Low sodium

Cancer (Neutropenic)

Delivery:
Provider delivers three times per week (typically four to six meals per delivery)

Open to Gold Coast MediCal members or MediCare
Private program buy direct
who had a hospital stay for diabetes or Congestive Heart Failure-related reasons within the past 30 days.
Must have service authorization from Gold Coast Health Plan Enhanced Care Management program
