Community Support: Medically Supportive Food/Medically Tailored Meals
Delivers fully prepared, medically tailored meals directly to individuals' homes, no matter how remote. Select meals from a variety of options. Designed by chefs and dietitians to support the nutritional needs of most common chronic conditions. Meals are packaged for convenient storage in the fridge for up to 14 days from delivery.

Delivery:
Provider staff or major delivery carriers (FedEx, UPS, GSO) delivers once per week (typically 10 to 14 meals per delivery)
Additional information:
Provides meals for members 2 years of age and older.
CalAIM Community Support Service - Medically Supported Food
Eligibility
Gold Coast MediCal members who had a hospital stay within the past 30 days.
Must have service authorization from Gold Coast Health Plan Enhanced Care Management program
. Chronic conditions
. Gestational diabetes and other high-risk pregnancy conditions
. Chronic or disabling mental or behavioral health disorders
. Patients discharging from the hospital or skilled nursing facility
. High risk of hospitalization or nursing home placement
. Care needs that require multiple doctors
Application Process
Submit a referral online https://www.momsmeals.com/communitysupports/inquiry-form/, Members will receive meals within 2-8 days after receiving the authorization from the health plan.
