Description
Provides a nurse follow-up for any family member who suffers an unexpected infant death related to SIDS. This includes:
. Assessment
. Education
. Case management
. Referrals to resources such as grief support and counseling
Also offers education and resources to any interested community group working with pregnant women and infants.
Eligibility
Open to all.
