Provides meals designed for specific medical needs and nutrition counseling for Gold Coast Medi-Cal members who have recently been hospitalized or have chronic conditions that could benefit from medically supportive food.

This is a CalAIM Community Support service under the CalAIM program.

Menu selections:
Four-week menu cycle
Customizable meals

Refrigerator/Freezer Requirement:
Refrigeration to store up to six meals at once

Delivery:
Provider staff delivers three times per week (typically four to six meals per delivery)
