Program: IN-HOME SUPPORTIVE SERVICES (IHSS)

Description
Helps low-income older adults and people with disabilities stay in their homes by helping them pay for support services. Services include:
- Light domestic help
- Non-medical personal care
- Assistance to and from medical appointments
Eligibility
Must be:
-Medi-Cal eligible
-If not already receiving Medi-Cal, individual will need to apply for it.
- Must also live in your own home or a dwelling of your choice.
-Blind, disabled or 65 years of age or older living in a home, apartment or residence of choice (not including a hospital, nursing home, assisted living or licensed care facility) and unable to live safely at home without care
Application Process
Begin the process by completing an IHSS application (form SOC 295). Applications can be found at vchsa.org/IHSS
A completed Health Care Certification (form SOC 873) signed by a licensed health care provider will also be required.
