Description
Offers support to Medi-Cal members facing complex challenges, including those with substance use or serious mental illness, individuals experiencing homelessness, and those with frequent hospital or emergency room use. Services include assistance connecting to housing and shelter resources, coordination of physical, mental health, and substance use care, support with medications, appointments, and transportation, linkages to food, benefits, and social services and personalized care planning.
Eligibility
Must be enrolled in Gold Coast Health Plan or under presumptive authorization for Medi-Cal in Ventura County.

Initial Assessment: A Conejo Health Navigator will conduct a basic needs assessment, verify ECM eligibility, and begin delivering immediate support services.
Comprehensive Assessment & Care Planning: Within the first 30 days, the Navigator will complete a comprehensive assessment, develop a person-centered care plan, and submit an authorization request to Gold Coast Health Plan (GCHP).
Automatic Initial Coverage: Members receive up to 30 days of ECM services automatically, regardless of authorization status.
Ongoing Engagement: Conejo Health plans for at least two meaningful touchpoints per month to support progress, coordinate care, and adjust the care plan as needed.
