Description
Provides enhanced care management services for the following adult populations of focus:
- High Utilizers
- Serious Mental Illness (SMI)/Substance Use Disorder (SUD)
- Individuals at Risk for Institutionalization, Eligible for Long-Term Care
- Child Welfare
- Individuals experiencing or at risk of homelessness
Eligibility
Must have Medi-cal and be associated with a Managed Care Plan (i.e. Kaiser, Blue Shield, Molina, HealthNet).
Application Process
Referral required, Care Coordinators will meet with the member to conduct a Risk Assessment to determine needs of the family.
