Description
Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services. Also coordinates client transportation for health care appointments.
Populations of focus-adults:
- Adults At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Serious Mental Illness (SMI)/Substance Use Disorder (SUD)
- Families or Unaccompanied Children and Youth Experiencing Homelessness
Eligibility
-Must have Medi-Cal coverage through Gold Coast Health Plan
-Need to make sure individuals don't have duplicate services, must know if individuals are in any other similar types of services; if so, they must unenroll in previous services if they want to participate in their services

Call for intake, self-referral and provider referral accepted
