Description
Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services. Also coordinates client transportation for health care appointments.
Population of focus-adults:
- Adults At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Adults with serious mental health and/or substance use disorder needs
Eligibility
Must have Medi-Cal coverage through Gold Coast Health Plan and or Kaiser Permanente
Application Process
Individuals may do a self-referral (must have full name, MediCal number, date of birth, address, phone number, and mention listed population of focus that they would fall under) or the health plan may submit one on their behalf
