Program: INTERFACE CHILDREN & FAMILY SERVICES - CALAIM ENHANCED CARE MANAGEMENT SERVICES (ECM)

Offers one-on-one, intensive case management services for GCHP members who may need extra support. Agency meets clients in the community. Services include employment search, housing search, connection to substance use disorder treatment, and many other needs. Also connects individuals to partner agencies to additional resources. Additionally, Interface is the only ECM provider in Ventura County that works with individuals who are currently incarcerated.

Population of focus-adult:

• Individuals Transitioning from Incarceration
• Individuals Experiencing Homelessness
• Individuals at risk for Avoidable Hospital or ED Utilization
• Individuals with Severe Mental Health and/or SUD Needs
• Adults Living in the Community at Risk for LTC Institutionalization
• Adult Nursing Facility Residents Transitioning to the Community
• Birth Equity Population of Focus
