Description
Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services. Also coordinates client transportation for health care appointments.
Population of focus-adults:
- Adults At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Adults with serious mental health and/or substance use disorder needs
- Child Welfare
- Individuals experiencing or at risk of homelessness
Eligibility
Must have Medi-Cal and be associated with a Managed Care Plan (i.e. Kaiser, Blue Shield, Molina, HealthNet, Gold Coast Health Care).
